- Individual – HEALTH “ACCIDENT” PLAN – $17.70/mo. – includes fixed-fee reimbursement benefits, unlimited use. Add Dr. visits rider for $1.20, providing reimbursement up to $200/visit. No limit on number of visits or benefit amount total.
- Individual – HEALTH SHORT-TERM plan – illness AND accident: – from $75/mo. $2,500 or 5,000 deductible, co-insurance 60/40 or 80/20. Lifetime MAX from $250,000 to 2,000,000.
- Group – HEALTH Major Medical plans: HMO, PPO, HSA available, employer & employee + dependents group coverage.
